Online Forms Instructions

Thank you for using Rank One Sport. You will use the student athlete’s school ID number to login. If this student athlete is returning from last year, please use the same login information you used previously. If this student athlete is a new participant in CCISD athletics (such as all incoming 7th graders), you will need to create a new account. If you have multiple children participating, you may use the same account to complete the online forms for each child. Upon approval of your completed online forms, you will receive email verification.

REQUIREMENTS FOR PHYSICALS FOR THIS YEAR

For the 2024-2025 school year, the Sports Medicine Department has decided to adopt the UIL guidelines regarding physicals expiring after two academic years, rather than just one.

This means that if your student will be in the 7th, 9th, or 11th grade during the 2024-2025 school year, it is required of them to obtain a physical before being eligible to participate in athletics.

It is required for ALL athletes and their parents/guardians to complete RankOne paperwork every year. This includes Medical History, UIL Forms, Emergency Form, and the Student Athlete Handbook Receipt. These must all be completed prior to your athlete being eligible to participate in athletics. If your student has a medical condition that requires long term care or athletic special circumstance, it is a requirement to obtain a pre-participation physical examination annually, regardless of the two-year rule established by UIL.

These forms may be completed on mobile devices as well. But it is more difficult when loading the UIL documents. Upon completion, please, do not turn in hard copies. You may print each document for your own records, or screen shot if you so choose.
